SASM Faith Formation Registration 2023/2024

Please note that you will have to complete a separate registration form for each child in your family.

Your parishioner number can be found on your envelopes. If you do not know your parishioner number, please call your home parish (St. Augustine's or St. Mary's) to find out your number. If you have not yet registered to be a member at one of our parishes, please go online to the parish website of your choice to register. Per diocesan guidelines, all Faith Formation families must be registered parishioners of a church.

For grades 1-5, when selecting their grade, be sure to also select the day that you'd like your child to attend.
Sunday morning classes meet at St. Augustine's two times a month from 9:15-10:15.
Monday evening classes meet at St. Mary's two times a month from 7:00-8:00.
As always, students are welcome to switch back and forth between class days, when needed, because of sickness or scheduling conflicts!

Please read the diocesan sacrament guideline below if you need to register your child for First Reconciliation and First Communion:

1. "The child is expected to have participated in parish Faith Formation or Catholic school religious education classes for at least one year before he/she begins the direct preparation for the Sacrament of Penance or the Sacrament of Eucharist."

2. "The child is expected to have been previously involved in the liturgical life of the parish faith community by regular attendance at Sunday Eucharist with his/her parents/guardians."

Will your child be preparing for First Reconciliation and First Communion this year? *note: unless your child attends Catholic school, he/she MUST also be registered for our bimonthly Faith Formation classes, as well.

2. "The candidate is expected to have been previously involved in both the liturgical and sacramental life of the faith community and in an on-going process of faith formation."

Is your child's baptismal certificate on file on your child's Breeze account? This is required for your child to receive the sacraments. If it is not, please add it as soon as possible.

After September 1, there will be a late fee of $10/child and/or sacrament automatically added.

Photo Authorization 2023/2024

Photographs and/or videos of minors are taken periodically for use in parish/parochial/diocesan publications, social media and/or websites in order to celebrate your child’s and his or her fellow students’ participation and accomplishments. In registering, you are acknowledging this and granting permission to St. Augustine's, St. Mary's, and/or the Diocese of Syracuse to use photos and videos of your child in our publications, displays, publications, social media and/or websites. We will NOT be posting names of children.

If you DO NOT WANT your child’s image to be taken and used during the current Faith Formation year, please mail a letter stating so to Mrs. Mackey at St. Mary's, 47 Syracuse St., Baldwinsville, NY 13027.
THIS REQUEST MUST BE MADE EVERY YEAR.
